Autopilot - how to smoothly transition between modes?

I'm trying to work up a sequence where I start with a follow, transition to a target and then transition back to follow. When I disengage autopilot to switch it resets the gimbal off my subject - I was wondering if there was some way to pick a mode directly while flying another to avoid that jump of the gimbal.
