Night Waiver

I am working on filling out a Night Waiver . Does anyone know where I can fins an example of a successful night waiver? Any tips on night flying would be appreciated. Thanks.

I have flown at night on a full moon, and ran the ISO to about 1000. Only flew it about 100 yds away, and had no problem see the red light s on the front of the Mavic. Also I lit up my landing pad with a really bright led shop light. I live in the high desert, on a 5 acre lot.
