No, I was never asked to fill out any information.Shucks, look like I won't get it tomorrow. Did you fill out the form asking for your SS#, etc? I did not but did fill out a shorter form asking for contact info.

That's the message I got from DHL for shipment on my Air 3s. I believe that does not mean it has been cleared by customs, just processed for clearance.
Where did you get the required form?My Air 3S has been stuck in Los Angeles Customs since Saturday October 19th. DHL customer service said CBP REQUIRES verification of my complete contact details, my SS number, and intended use of the product. The Government already knows most of this, but I must verify everything! DHL said the information will be handled in the strictest security not shared by anyone but CBP. I asked a lawyer friend — he said it’s legit, CBP can request this information anytime they desire to…. So, I gave it to them on a secure site. DHL then said all requested information was provided to CBP, there’s nothing more I, or they, can do besides wait….
“All documentation for this order has now been submitted, and your shipment is now pending review to clear UNITED STATES OF AMERICA Customs. You will be contacted if any further issues arise, but no further action is required at this time.‘

Great! Unfortunately, all I have is a digital client copy of my 1040, and it has the SS# XXXXXX. I lost my card decades ago. When you apply for a replacement, SSA practically pleads with you not to get one, you really don’t need a physical card, So, I never got one. I don’t think you need the form, you need to provide DHL, who will give to CBP, a document providing your name, address and SS#. Copies of the top of your 1040, w/o financials works. SS card, or W-2 works. Black out financials. I don’t have these (long story, ), so I tried another document. I’ll know if it works on the 25th…Where did you get the required form?
